HIPAA Privacy Notice

Effective date: July 17, 2026

Gold Hands Home Care is required by law (HIPAA) to maintain the privacy of your protected health information (“PHI”), to give you this Notice of our legal duties and privacy practices, and to abide by the notice currently in effect. We will also notify you following a breach of unsecured PHI.

How we may use and disclose your information

Treatment

We use your PHI to provide your care (for example, our nurses share information with your treating physician or coordinate your therapy).

Payment

We may use and disclose your PHI to bill and collect payment for services (from you, your insurer, or Medicare/Medicaid).

Health care operations

We may use your PHI to run our agency and improve quality (internal evaluations, training, audits).

Other uses permitted by law

• When required by law (public health, health oversight, court orders, safety).
• To prevent a serious threat to health or safety.
• To family members or others involved in your care, unless you object.
• Appointment reminders and treatment alternatives.

Uses requiring your written authorization: marketing, sale of PHI, most uses of psychotherapy notes, and any use not described in this notice. You may revoke an authorization at any time in writing.

Your rights regarding your information

• Inspect and obtain a copy of your record (normally within 30 days; a reasonable copy fee may apply).
• Request corrections if you believe information is wrong or incomplete.
• Receive an accounting of disclosures made outside treatment, payment and operations for the past 6 years.
• Request restrictions on certain uses or disclosures (we must agree to a restriction to your health plan if you paid for the service in full out of pocket).
• Request confidential communications by reasonable alternative means or locations.
• Obtain a paper copy of this notice upon request, even if you agreed to receive it electronically.
• Choose someone to act for you (legal guardian or health care power of attorney).

Complaints

If you believe your privacy rights have been violated, you may file a complaint with our office (details below) or with the U.S. Department of Health and Human Services, Office for Civil Rights (OCR): 200 Independence Avenue S.W., Washington, D.C. 20201, phone 1-877-696-6775, or at www.hhs.gov/ocr/privacy/hipaa/complaints. We will not retaliate against you for filing a complaint.

Changes to this notice

We may change this notice; the current version will be available at our office and on this website.
