Gold Hands Home Care is required by law (HIPAA) to maintain the privacy of your protected health information (“PHI”), to give you this Notice of our legal duties and privacy practices, and to abide by the notice currently in effect. We will also notify you following a breach of unsecured PHI.
We use your PHI to provide your care (for example, our nurses share information with your treating physician or coordinate your therapy).
We may use and disclose your PHI to bill and collect payment for services (from you, your insurer, or Medicare/Medicaid).
We may use your PHI to run our agency and improve quality (internal evaluations, training, audits).
Uses requiring your written authorization: marketing, sale of PHI, most uses of psychotherapy notes, and any use not described in this notice. You may revoke an authorization at any time in writing.
If you believe your privacy rights have been violated, you may file a complaint with our office (details below) or with the U.S. Department of Health and Human Services, Office for Civil Rights (OCR): 200 Independence Avenue S.W., Washington, D.C. 20201, phone 1-877-696-6775, or at www.hhs.gov/ocr/privacy/hipaa/complaints. We will not retaliate against you for filing a complaint.
